Madrid control centre operator sent Spain's deadliest train crash into motion
A Renfe high-speed service struck a stationary Iryo train near Toledo after an operator authorised it to continue with the LZB safety system offline. Spain's Transport Ministry has raised the death toll to 69, with about 80 people injured.
